Retractions

When we're wrong, we publish it.

A retraction is a linked DecisionRecord that supersedes the original. The record stays in the ledger; the amendment explains what changed and why. Below are two recent retractions in full.

08 Apr 2026
Original claim

AP delay — offered an evidence-first probe with a 14-day monitoring window. Confidence 0.71.

What actually happened

Customer had opened a contract dispute Nooterra had not surfaced. The evidence drawer missed a Zendesk ticket filed outside the AR-tagged channel, so the hidden-world search treated the case as a simple AP delay.

What changed

Dispute-classification rules expanded to include any open ticket on the contract entity, not just AR-tagged tickets. The scenario envelope now carries a lower fidelity label when support coverage is partial.

Original claim

Relationship-safe offer — Net-45 with 1.5% late-fee waiver. Stated high confidence.

What actually happened

Post-hoc validation showed the relationship-safe policy family had weaker outcome support than the Trust Ledger entry implied. The recommendation shipped; the stated confidence was overstated versus observed.

What changed

Per-policy-family fidelity labels were added to the nightly review loop. The retraction record publishes the delta between stated support and observed support so the drift is legible.
